Presentation:
Mary Blogs was referred by her GP on 10th January 2025 for psychological treatment of anxiety and panic symptoms. Treatment commenced on 15th January 2025, with Mary Blogs presenting with frequent panic attacks and generalised anxiety that had been impacting her work and social functioning for approximately three months.
Mary Blogs reported that her symptoms began following a workplace restructure in October 2024. She described experiencing panic attacks 3-4 times per week, characterised by rapid heartbeat, shortness of breath, and fears of losing control. Mary Blogs stated that these symptoms were significantly impacting her ability to attend work meetings and social gatherings. She reported living alone, working as an accountant, and having limited social support apart from her partner of two years. Mary Blogs’s initial treatment goals included reducing the frequency of panic attacks and returning to normal work and social functioning.

Treatment:
Mary Blogs attended weekly sessions from 15th January 2025 to 4th June 2025, completing 18 sessions to date. Treatment utilised cognitive behavioural therapy (CBT) approaches focusing on panic disorder and generalised anxiety. Specific interventions included psychoeducation about anxiety and panic, breathing retraining, progressive muscle relaxation, cognitive restructuring, and graded exposure exercises.
Mary Blogs was assigned homework tasks including daily relaxation practice, thought monitoring, and behavioural experiments. She demonstrated consistent engagement with treatment, attending all scheduled sessions and completing assigned homework tasks. Mary Blogs reported practicing relaxation techniques daily and successfully implementing cognitive strategies in anxiety-provoking situations.
Mary Blogs demonstrated significant progress toward her treatment goals. She reported a reduction in panic attack frequency from 3-4 times per week to once per week by session 8, and no panic attacks in the past month as of her most recent session. Mary Blogs stated that she had successfully returned to attending work meetings and had resumed social activities including dining out with friends. Her DASS-21 scores showed improvement from severe to mild anxiety symptoms.
Mary Blogs’s most recent clinical presentation on 4th June 2025 showed continued stability in mood and anxiety symptoms. She reported feeling confident in managing anxiety symptoms and had not experienced any panic attacks in the previous four weeks. Mary Blogs stated she was functioning well at work and in social situations. Treatment is currently in the maintenance phase with sessions scheduled fortnightly.
The current treatment plan involves continued fortnightly sessions for the next month, followed by monthly maintenance sessions. Mary Blogs has expressed satisfaction with her progress and confidence in maintaining her improvements independently.
Recommendations:
Continued psychological treatment on a maintenance basis is recommended to consolidate gains made during intensive treatment. No additional referrals have been made at this time.
