Assessment:

Mary appears stuck in contemplation stage regarding lifestyle changes discussed previously.
Response:
Cognitive restructuring techniques applied to challenge catastrophic predictions about performance review. Mary engaged well and identified alternative perspectives with prompting. She showed emotional relief when reframing worst-case scenarios. Mary expressed frustration about difficulty implementing skills independently between sessions. Motivation appeared moderate with some ambivalence about homework commitment.
Plan:
Mary agreed to complete daily thought records focusing on work-related worries. Plan to continue CBT approach with increased focus on behavioural activation. Next session will review performance review outcome and practice assertive communication skills. No referrals needed at this time. Next appointment scheduled for 22 March 2024 at 2:00 PM.
