A comprehensive observational summary template for the Autism Diagnostic Observation Schedule, Second Edition (ADOS-2) Module 1. This module is designed for children 31 months and older who do not consistently use phrase speech.
This assessment session template captures behavioural observations across communication, social interaction, play, and restricted/repetitive behaviours during the standardised assessment activities, supporting diagnostic formulation using neurodiversity-affirming language balanced with standardised clinical terminology.
It is recommended that clinicians include their behavioural observations of the client as a note or added context, prior to generating the summary.

Based on today’s structured observational assessment using ADOS-2 Module 1, Sam demonstrated a pattern of differences in social communication and reciprocal social interaction, along with restricted/repetitive behaviours and interests, that are consistent with an autistic presentation. Specifically, he showed absent declarative pointing and showing, limited integration of gaze with other communicative behaviours, minimal spontaneous joint attention, absent pretend play, and prominent repetitive behaviours including spinning and object-lining. These observations should be integrated with developmental history, caregiver report, and other assessment data to inform the clinician’s diagnostic formulation.
