The following report is an update on my therapeutic work with Mary Blogs.
Mary has been engaging in weekly individual psychology sessions for the past six months, utilising cognitive behavioural therapy and dialectical behaviour therapy approaches.
Background
Mary has a primary diagnosis of Borderline Personality Disorder with comorbid Major Depressive Disorder and experiences chronic pain related to fibromyalgia.
She has a history of childhood trauma and self-harm behaviours, with previous psychiatric hospitalisations occurring in 2019 and 2021. These experiences have significantly impacted her ability to maintain stable relationships and consistent daily routines.
Mary currently lives independently in supported accommodation and receives weekly assistance from support workers with meal planning and medication management. This arrangement provides appropriate structure while promoting her independence.
Over recent months, she has demonstrated improved emotional regulation with self-harm episodes reducing from daily occurrence to approximately once weekly. She has successfully implemented distress tolerance skills during crisis situations.
Mary actively participates in therapeutic activities focused on developing emotional regulation and interpersonal effectiveness skills. She has increased her community engagement, including regular attendance at a local art class, which has improved her capacity to manage her psychosocial disability.
Functional Impact of Psychosocial Disability
Mary’s Borderline Personality Disorder significantly impacts her daily functioning through emotional instability and interpersonal difficulties, which affect her ability to maintain consistent routines and stable relationships. Her chronic pain condition exacerbates emotional dysregulation and contributes to periods of social isolation.
Current Capacity-Building Strategies
Current therapeutic interventions focus on dialectical behaviour therapy skills training, particularly in the areas of distress tolerance and emotion regulation. Mary demonstrates strong engagement with therapeutic work and has developed excellent rapport with the treating clinician. She consistently completes homework tasks and has successfully implemented new coping strategies, including the use of grounding techniques during panic attacks and contacting support services during depressive episodes rather than isolating herself.
Recommendations
Continuation of weekly individual psychology sessions for the next six months is recommended to consolidate therapeutic gains. A referral to occupational therapy would be beneficial to address daily living skills and explore vocational opportunities. Consideration of additional mental health support worker hours during high-stress periods would provide valuable community-based skill reinforcement.
Conclusion
Mary has made significant progress in developing emotional regulation skills and reducing self-harm behaviours throughout the therapeutic intervention period. She has demonstrated increased capacity for community engagement and improved crisis management skills.
