A treatment plan records the diagnostic impression, presenting problems, goals and interventions agreed with a client, along with the measures used to monitor progress toward those goals.
This report template synthesizes information from NovoNote and NovoPsych psychometrics to assist clinicians in preparing a draft treatment plan. It captures the diagnostic impression with code, mental status examination, presenting problem, administered measures, and numbered goals with measurable objectives, named interventions and target completion dates, together with treatment frequency, barriers, provider coordination, discharge criteria and risk.
It can be used as a session note for sessions dedicated to treatment planning or review, or generated as a formal document from progress notes, intake and assessment records.
