Emma has made substantial progress over the course of treatment. Intrusive memories have reduced to below clinical threshold frequency and she reports that when they do occur she is able to manage them effectively using her grounding and self-regulation skills. Sleep has improved significantly, with nightmares now occurring only occasionally. Hypervigilance and emotional numbing have largely resolved. Emma describes feeling more like herself than she has in several years and reports a meaningful improvement in her overall sense of wellbeing.

Emma has returned to full-time teaching and reports that she is managing her work environment with confidence. Goal description: Reduce the frequency and intensity of PTSD intrusive symptoms to below clinical threshold on the PCL-5.
What progress has been made towards achievement of this goal?
Clinical progress: Emma has achieved this goal. Intrusive memories, which were occurring daily at the commencement of treatment, now occur rarely and are managed effectively when they arise. Nightmares have largely resolved. Emma demonstrates consistent and independent use of grounding strategies across a range of contexts.
Outcome measures: PCL-5 — administered 06/01/2026 (baseline): Total score 48 — Severe range. Administered 12/05/2026 (completion): Total score 18 — below clinical threshold (below 33). This represents a reduction of 30 points and indicates full symptom remission across all PTSD symptom clusters.

Goal description: Improve capacity to attend work full-time without significant trauma-related distress.
What progress has been made towards achievement of this goal?

She reports that her pre-work grounding routine has become habitual and that she no longer finds her work environment consistently triggering. Occasional difficult moments occur but are managed independently and do not significantly impact her functioning.
Outcome measures: None administered for this goal. Self-reported work attendance and distress ratings reviewed across the final four sessions confirmed sustained full-time attendance without clinically significant distress.

She is engaging regularly in social activities with friends and family and reports that avoidance of interpersonal situations has largely resolved. She continues to attend her walking group weekly and has expanded her social activities to include occasional larger group settings.
Outcome measures: None administered for this goal. Self-monitored social activity log reviewed across final sessions confirmed consistent engagement in three or more social activities per week.

4. Next Steps
Self-management plan:
Emma and Dr Green collaboratively developed a self-management plan prior to the completion of treatment. The plan includes daily use of grounding and mindfulness strategies, use of her pre-work routine on high-stress days, and continued engagement with her walking group as a protective social activity. Emma has identified early warning signs of symptom return, including sleep disturbance and social withdrawal, and has a clear plan to contact her general practitioner promptly if these emerge. A letter has been provided to Emma’s GP summarising treatment outcomes and recommending a low-threshold re-referral pathway should symptoms recur.
